NASAL sprays gave Ms Shu Lee blessed relief from a chronically stuffed nose, but being able to breathe freely came at a shocking price. She became addicted to these sprays.


A polyclinic doctor had recommended them to alleviate her lifelong nasal problem. Available without a prescription and costing only $10 a bottle, they seemed an obvious option. The effect was dramatic.

'This wonder drug helped me breathe properly for the first time in my life,' said Ms Lee, a 27-year-old advertising executive.

The spray relieves blocked noses instantly - but only temporarily - using a highly addictive chemical compound called oxymetazoline, the main ingredient in popular brands Afrin and Iliadin. When its effect wanes, it causes rebound congestion so users must inhale more to get relief, hence developing a dependency.

Ms Lee was soon inhaling four times a day - twice the maximum recommended dose.
And although the label said the spray must not be used more than three days in a row, she progressed to buying a bottle every month.

'I took it everywhere I went,' she said, adding that in the three years she was addicted to it, she spent more than $350 on her habit.

Experts estimate that at least half the 800,000 Singaporeans who suffer from chronic nasal congestion have been dependent on these sprays at some point. Extreme dependencies may even require surgery to cut away or shrink swollen nasal tissue. Severe addiction, which may cause side effects such as paranoid psychosis, is estimated to affect 1 per cent of patients getting ear, nose and throat (ENT) treatments. But doctors say the real numbers could be far higher, as many people are unaware that they are addicted.

ENT specialist Adrian Saurajen, who is based at Mount Elizabeth Medical Centre, said: 'When nasal sprays are used continuously for more than a week, they can cause dependency.'

Reported cases are rare here, but there is concern because Singapore has the highest incidence of allergies in the world, said ENT specialist Gerard Chee, of the G H Chee Ear Nose Throat and Dizziness Centre.

Ms Lee went cold turkey when she realised the extent of her addiction, going through 'two months of sheer agony and three months of discomfort' before she got over it.

'I had sleepless nights from the congestion. Sometimes I couldn't breathe through my nose even in the daytime,' she said.

That was more than four years ago.

Since then, her sense of smell and taste has largely returned, and - oddly enough - she does not suffer from nasal congestion any more.

'I'm just glad I kicked the habit,' she said.

I personally feel that the report could be worded better with some sensitivity - not because I support any drug industry, but rather use of words such as "addicted" can really hurt a person's life.

Let's say if the person's family members are not very well-informed and they jump at catchwords thrown to them by the media such as the word "addicted" especially when they are connected with the word "drugs" - imagine the chaos that can result. This may sound extreme but then a person's life may just fall apart when they are ostracised on par with heroin due to people who jump at certain catch words instead of seeing the whole picture.

A lot of these products can be sold anywhere and bought anywhere, take for example Afrin Nasal Spray - note that it is GSL (General Sales List) - even your Chinese Medicine Hall can stock it for sale.

Oxymetazoline and ephedrine sprays are examples of decongestants and these are not meant for long - term usage. They should be used max for 3 days with a break of few days in between (drug holiday). If this spray (under normal dosing), it's time to see doctor as it could be something more serious and need long-term treatment or alternative forms of treatment.

Miss Shu Lee's case is a case of treatment failure. She should have followed up with a doctor or pharmacist instead of going on her own accord to overdose herself with oxymetazoline. It is not advisable to use the decongestant sprays for so long. The relief from such sprays is only temporary - in some practices, these sprays are used to unblock the nose before applying other sprays for long-term use such as corticosteroids, beclomethasone, mometasone and fluticasone.

If she returns to her doctor, she may be directed to use a corticosteroidal spray that may cost more but with a better safety profile. These sprays are also used prophylatically so you won't have much of sudden onset of nasal blockage.

The doctor may order some lab tests to check whether she has certain allergies - in this way she will know what triggers to avoid. X-rays and CT scans may also be warranted if the blockage is very often - to check for anatomical problems where surgies may be carried out.

Non-pharmacologic methods to help includes using inhalation and maintaining a clean sleeping or working or home environment. Some people get nasal blockage at certain places - work, home or school but as soon as they leave, the blockage stops. This may be due to the aircondition of the place and may mean that the aircon need cleaning. The same thing applies if the person wakes up with a blocked nose - he/she may want to change and/or clean the bedsheets and mattress as well as pillows - these may be due to dust mite allergies. Mites from different regions give different allergy profiles - when you travel overseas (even to dusty places), you may realise that you may not get the stiffles despite the dust.

Tamtam's new puny plant with big flower is Haraella retrocalla syn. Haraella odorata. Best given strong sunlight to flower when the plant is established on its mount. When given strong light, the fragrance tends to come out

Repotting Cattleya walkeriana

These photos are a bit old and this precious plant was lost sometime ago due to a massive attack of webspinners (destroyed about 1/2 of my collection TMDKKJ)

The company -Hirota International Flowers - had since closed down after the World Orchid Conference Djion, France. Bought as a seedling until it grew too big for the pot - Cattleyas can flower better when they fill the pot and start sticking out but when they stick out too much - they may dehydrate easier because of lack of water for the dangling roots. The media in which the start of the plant is in may have very few viable roots or the olf media had already started rotting.

Your cutter and lighter - the cutter is for duh and the lighter is to sterilise the cutting blades. The key thing about sterilisation is the removal of viruses - these are systemic vectors and are almost impossible treat. Bacteria and fungi infections are not too bad as they can be dealt with very easily.

Pots - try to get those with holes a plenty. Although the roots need water, they also need to breathe. They also provide better drainage - but note that these holey pots are not suitable for those with a very dry home environment.

Powdered sulphur is the fungicide / dressing of choice for me - convenient and potent. Some prefer to use cinnamon but I do not quite the cost and the smell (it is not on something edible for goodness sake!). Captan or any other commercial fungicidal powders will also suffice.

Sphagnum moss is a moistening agent just to help retain water for longer periods of time. Good sphag has long fibres and are free of other debris such as leaves and stems. Other alternatives include coir (coconut husk fibres) and osmunda.

These are fernroot cubes - it can help retain water and also provides some form of nutrients for the plant. They can be made by sawing down fernroot slabs into size.

Charcoal is for drainage and it also helps to absorb excessive salt - in fact when repotting old plants, you may see shades of white salt deposits on the charcoal surfaces. The salt deposits also indicate the need to change the potting media.

Cut the plant at the appropriate length. You should be left with a matured plant with about 2-3 backbulbs and one lead. The other backbulbs in the pot may be discarded (they are very slow growing at this stage - they are exhausted) but if you have the patience and space, you may want to repot them and try growing them.

Dust the exposed rhizome with a fungicide

Trim down the roots and dust the exposed ends with funigicide. If the roots are too long, there is a problem to tucking the plant into the pot and the roots may end up breaking anyway.

This pot is too small for the new plant - there is not enough space to accommodate new growth even though it is placed at the edge of the pot.

This pot can take another 2-3 pseudobulbs and is appropriate. If the pot is too big, the media may stay too wet as there is too much water and too little utilised by the plant. Also, if the pot is too big, it may take too long for the plant to flower - due to the Cattleya's habit of flowering only when the pot is compacted with pseudobulbs or when they start growing over the edge.

I like to pad the bottom of the pot with a water retaining agent as relative humidity is more important to epiphytic roots than direct contact with water.

Next is crocking material - to provide lift to the plant so as to raise it to above the rim of the pot.

Fit your plant in. Try to force the backbulbs to the edge of the pot - it allows the space in the pot to be utilised fully.

Top up with fernroot cubes and try to stabilise the plant. If the plant is still wobbly, you may want to introduce a stake.

Tag the plant as always - if not you'll have a hard time identifying the plant if you have many copies and they look so similar. Some people may want to write the date of repotting but I repot at an ad hoc basis based on regular inspection, so I don't find a need to do so. Some may also write the name of the original supplier on the label.

"The name of an orchid consists of two parts, its genus and its epithet. Using the name of the world's largest orchid species, Grammatophyllum speciosum, as an example, Grammatophyllum is the genus and speciosum is the epithet. The term 'genus' is used by taxonomists to indicate a certain group of orchids which share some common characteristics. Usually there is more than one member in a genus, and each of these members has its own epithet. When the name of a species is written, the genus always begins with a capital letter, whereas the epithet is always written in small letters. Varieties:
Varieties of orchids come in 2 forms: the natural occuring variety and the cultivated variety. The natural occuring varieties usually comes in different colour forms or different sizes of plants. E.g. Laelia rubescens var. alba and Laelia rubescens var aurea. Notice that the "var." is not italicised.
Cultivated varieties or cultivars are names a breeder or collector gives to his/her plants to differentiate them from the other plants of the same species or hybrid. These may be plants of superior quality or have some sentiment value attached. These plants may or may not be awarded but all awarded plants can be given a cultivar (NB - not all good plants are awarded and neither are all awarded plants are necessarily good). This names are usually the last component of a plant's name and is in single quotes (''). E.g. Fredclarkeara After Dark 'Sunset Valley Orchids'

Hybrid names:

Can be anything under the sun as long as they follow the rules of the registrar in Handbook on Orchid Nomenclature and Registration. These names can range from Sogo Cock to Stone Addict as long as they follow the word limit (sorry can't remember).

But it is good to note that every progeny of a hybrid will not carry the same cultivar unless they are mericloned. I.e.:

Not every Mormodia Painted Desert X Catesetum Donna Wise = Fredclarkeara After Dark 'Sunset Valley Orchids'

However, the reverse is true.

Video game addiction in children and teenagers in Taiwan is associated with levels of animosity, social skills, and academic achievement. This study suggests that video game addiction can be statistically predicted on measures of hostility, and a group with high video game addiction has more hostility than others. Both gender and video game addiction are negatively associated with academic achievement. Family function, sensation seeking, gender, and boredom have statistically positive relationships with levels of social skills. Current models of video game addiction do not seem to fit the findings of this study

How to create new mounts

For your epiphytes - seedlings or matured plants.

Mounts can be any material from styrofoam slabs, fernroot, driftwood, cement-coated styrofoam or even sheet metal. - You need to know what the plant likes when whether it is suitable for your micro-environment.

When positioning the mount, think about which side your mount is facing and how it is going to hang - this will determine how you shape your hook.

Mount sizes and shapes can also vary - if your mount is big, chances of it remaining wet for longer of periods of time is also there - which can mean higher chances of rotting. If your mount is small, it may dry out quickly and also means there is a call for frequent repotting when yur mount is totally covered by the plant.

Tools of the trade:

Stiff straight wire (same as those staking wires which the Taiwanese use, very sturdy yet can still be bent into shape),
Straw string (my preferred tying string for orchids - weather resistant, slightly stretchable, easy to cut)
Fernroot slab (you may cut into your standardised size; you may also use whatever mounting materials you want, the concept is about the same)
Sphagnum moss (Argentinian, Canadian, Australian or New Zealand. try to avoid those imported from China - a lot of muck to remove and can look very digusting. The main function of the moss is to hold water. Coconut husk, osmunda are good substitutes,)
Drill (use the finest drill bit possible)

Take your wire and cut to appropriate length

Drill a hole in your mount. Use a fine bit. If the hole is too big, it will cause a lot of pivoting for the mount - especially when there is strong wind or when you move your plants a lot - it will minimise physical damage.
